Who is eligible to apply for services?
ONLY individuals who are on a waiting list for waiver services currently provided by Peak Community Services.
What services will be eligible?
ONLY Services for which there is a BDDS waiting list. Currently, those programs include Family Support Services,
Supported Community Living, and Work Services.
How do I know if I am on a waiting list, or where I am on that list?
Individuals receive a letter notifying them that they are ON a waiting list. They do not receive any updates in writing.
What does this mean for the Foundation?
Individuals receiving services supported by the Foundation will have a contractual obligation to the Foundation
regarding their status on the waiting list. They cannot take themselves off the list or refuse support once they come to
the top of the list. This will be a closely-monitored by the individual's QMRP.
Can someone who is already a consumer be receiving services in one area and be on a waiting list for other services?
Yes. We have consumers receiving services from the agency who are also waiting to receive waiver services.
Will funds be available for current consumers?
The Foundation will provide funding for services to individuals who are on a waiting list to receive government funding for
support. If current consumers are receiving services in one area and are on a waiting list in another, they are eligible to
apply for services and will be considered with all other applicants based on the criteria established by the Foundation.
Will funds be available for all of the programs Peak Community Services provides now?
Some of the programs we provide do not have waiting lists; services are necessary and the government provides payment.
The Foundation will not provide services to individuals for these programs. (i.e. SGL & TRIP)
